Appointments

SHS is an appointment-based clinic. You will usually be seen within 24-48 hours, but this is dependent on the patient flow of the clinic, which varies according to day and time of year. When available, students will be scheduled with a consistent primary care practitioner.

To schedule an appointment, call (773) 702-4156 between 8am and 5pm. If your situation cannot wait until business hours, please review information about Urgent/Emergency Care.

 Treatment of Minors (under age 18)

A parent or guardian must provide consent before a minor may be seen at the Student Health Service. Please complete this form and return to SHS by mail or hand-delivery.
